COVID-19 Information and Visitor Restrictions:
Here at Noblesville Urgent Care, we are still committed to providing excellent care. All visitors to our facility must wear a mask. If you do not have one, we can provide one to you. However we, are limiting the number of visitors to just the ill person that needs to be seen, unless they are a minor and we are asking for only one parent to attend this visit.
During this time, we are still seeing patients on a walk-in first come first serve basis. Please review our instructions on how to visit our facility.
VISITING OUR FACILITY
If you are ill such as fever, cough, or respiratory symptoms and or been exposed to COVID-19, or wishing to be tested for COVID-19, or traveled outside of the country in the last 30 days please download the forms below and fill those out prior to coming to our facility. If you cannot print those out prior to a visit, forms are available at our facility. The forms are in the information white box at the back of our building.
1) Retrieve the forms and return to your vehicle to fill those out.
2) Once you are finished filling those forms out place them along with picture id and insurance cards in that white information box, then proceed back to your vehicle.
3) Once securely back in your vehicle call us to let us know they are securely in the box.
4) Then when we have retrieved those and scanned in your documents, we will then phone you once we have a safe and secure room for you.
If you have filled out the forms prior to arriving, then place them in the box as instructed above (you do not need to fill the forms in the box out).
COVID-19 Rapid Testing
Rapid COVID-19 Antigen testing is only being performed as a part of an urgent care visit at this time. When you’re seen by one of our providers they can determine if a COVID-19 nasopharyngeal swab is needed. We will bill your health insurance. However, those who do not have health insurance (self-pay) will incur the following costs for the test.
COVID-19 Antigen testing CPT 87426 – $100
Rapid COVID-19 Antigen testing has not been approved by the FDA. Because there are currently no FDA-approved tests, the FDA has authorized certain tests under Emergency Use Authorization (EUA) if they meet certain criteria.
We are currently using the Sofia SARS CoV-2 Antigen test, which has been authorized by the FDA under the Emergency Use Authorization (EUA) for use by authorized laboratories and providers.
